Dr Conner of the Sleep Disorder Center said Thursday, 4/14/21 that he would place an order for me with you for BiPAP masks and machine. I am wondering if you got the request. Also I believe you have my insurance information, Medicare, secondary (Blue Cross), and RX (Optum RX). If not, I would be happy to give you that information. I live at Villa Maria,3905 Kingston St to which I understand you deliver. You also supposedly have insurance information on my wife, Willene, We plan to make you the pharmacy of record with our medical providers. I look forward to hearing from you.
